No, she was seen later on in the episode. So he did allow her to live.

Faceless Girl? Was seen after the scene where he defaces her? I don't
think so but i'd be happy to hear a cite.

Okay, downloaded the ep and edited out the relevant parts. It's a diff

    Those were two different crewmembers.
As evidenced by the fact that the second crewwoman had no trouble
    walking until Charlie froze her.

the Potemkin or the Yorktown (I think it was the Potemkin) carries the

named that. You remember, David Gerrold named Sherman's Planet after
his girlfriend, so I'd like to think there's a similar story here.
There's a Huron, CA and there have been a couple of ships named Huron,
    but none particularly famous.

Gerrold's on Facebook; you could always ask him there.

We didn't see Roddenberry either, even though he was the ship's cook.
I guess the turkeys changed back too. Sure hope they didn't have
anything that anyone was allergic to.
I guess Janice Rand's perfume disappeared too, right off her body.

    It's a great episode, but
just once in a while, you'd like to see them show a successful but

There were a couple of TOS episodes with messy endings:

    1. "Charlie X": The Thasians take him back to his life of loneliness
after telling Kirk there's no other solution

It was implied or stated that everything on the Enterprise was returned to normal (it was too late for the Antares and crew.) Sam was returned (probably to the gym), Janice was returned (to the bridge for dramatic effect) and the girl got her face back.

I really think it would be too late for her too unless he put a nosebon the back of her head or something.

It's not discussed in the episode,
but I'm sure that 23rd century medical technology could have kept her
alive on life support while McCoy figured out what to do with her. Even today, a tracheotomy could get oxygen into her lungs and so on.

They were able to keep Capt. Pike alive, despite the fact that virtually
his entire body south of his neck was useless.
